If you’re a Priority Health member through Corewell Heath, you can lower premium cost by enrolling in Healthy Lifestyles. With enrollment you can choose between two different Healthy Lifestyles rewards both providing up to $650 per year. One reward option is the medical premium credit of $25 per pay period, up to $650 per year. The second reward option is enrollment in a Lifestyles Spending Account (LSA) with $650 a year for reimbursement of well-being expenses when you enroll in a medical plan.
Healthy Lifestyles requires a separate enrollment outside of benefits enrollment. All new hires are given 90 days from date of hire to complete enrollment in this program to receive any program rewards.
All new hires who enroll in Healthy Lifestyles will automatically receive the premium credit when enrolled in a medical plan. Any rewards are applied once enrollment in the program has occurred.
In support of the mental well-being of our team members and their families, we offer behavioral health virtual care covered at 100% before deductible in the HMO and after deductible in the HDHPs.
Team members have flexibility to choose where they fill prescriptions. You can save on your prescription drug costs by filling with tier 1 medications or by using the mail order prescription program through Express Scripts.
The basic and enhanced dental plans provide team members with comprehensive preventive, diagnostic and basic and major restorative services benefits. The enhanced plan includes orthodontia coverage for children and adults.
Team members have two vision plans to choose from.
Full-time and part-time benefit eligible team members receive a core short-term disability and long-term disability benefit at no cost.
Everyone has diverse needs. Consider enrolling in one (or more) of the many voluntary benefit plans. Enrollment for Pet Insurance, Auto and Home Insurance will be done through Aon Everyday Marketplace. Team members can enroll in Group Accident, Group Hospital, Group Critical Illness, Identity Theft Protection and Group Legal through Workday.
Things to Remember
You don't have to enroll in every benefit option. For example, you can enroll in vision coverage and life insurance but choose to waive medical and dental coverage.
If you want to make a large, one-time contribution to your HSA, you will make that contribution directly to HealthEquity.
You need to have documentation for new dependents, including spouse, available for enrollment.
Tiers within the Corewell Health medical plans are not something you enroll in or are locked into—it's how your deductible is applied, based on where you choose to receive your care and the provider you select.
Consider planning for your future. Be sure to contribute to the retirement plan to take advantage of the matching contribution.
The Employee Assistance Program (EAP) offers you and your immediate family multiple resources. This program is through Ulliance.
A note on any supplemental life and/or spouse life elections:
Guarantee issue is the amount of life insurance a team member can obtain for themselves and/or their dependents, when first eligible for coverage, without the proof of good health, otherwise known as evidence of insurability (EOI).
Guarantee Issue:
Supplemental life has a guarantee issue amount of the lesser of 3 times annual salary or $750,000, any amount elected over the guarantee issue will be subject to EOI.
Spouse life has a guarantee issue amount of $60,000, any amount elected over guarantee issue will be subject to EOI.
Child life, there is no EOI for child life.
When first eligible for coverage, new team members, newly integrated team members and newly benefit eligible team members do not have to show EOI for amounts below guarantee issue for supplemental and dependent life insurance coverage you choose to elect. If you decline coverage when first eligible, you will need to show EOI to obtain coverage in the future.
There is a buy-up long-term disability (LTD) option. Eligible team members will have a one-time opportunity to enroll for buy-up LTD insurance without proof of good health. Any future elections will require proof of good health. A review of pre-existing conditions may apply to your elected coverage. Please refer to your Plan Summary or Plan Certificate for more details.
